What to Expect at our Scottsdale Dental Office
Your First Visit
We’ll reserve about an hour for your initial exam with Dr. Clark. After reviewing your paperwork and discussing any concerns you may have, Dr. Clark will thoroughly examine your teeth and soft tissues. All X-rays will be taken digitally to minimize radiation exposure. Once Dr. Clark gathers all of your information, he can provide an evaluation that will outline your existing dental problems and proposed treatment. In most cases, we can provide a professional teeth cleaning, including oral hygiene tips and suggestions for home care, at this time as well.

For your convenience, our new patient forms are available online. Please print them, fill them out, and bring them with you to your appointment. This will help save you time during the check-in process.

Financial Information
Dr. Clark is committed to providing affordable dental care without sacrificing quality! Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Please be prepared to pay your estimated portion at the time of service.
We accept Visa, MasterCard, American Express, debit card, cash, check or money order.
If you would like to extend your payments over a longer period of time, various financing plans can be arranged through CareCredit, Chase Health Advance, or Citibank. Depending on the amount, these lines of credit are up to 12 months, interest- free.
We offer a 5% discount on charges that exceed $100 of the total amount due, if you pay in full at the first treatment visit.
Please ask a member of Dr. Clark’s staff, and we will review our payment options to accommodate your financial needs. If you’d like to make special arrangements, please talk to Traci prior to receiving service.
Dental Insurance
Our office understands the value of insurance benefits, and we will gladly process and file your claim free of charge. Simply bring your estimated portion at the time of service. Please understand that this is only an estimate based on the information available to us. The final obligation for treatment is between you and our office. We will assist you in any way that we can; however, there are no guarantees and payment is due regardless of the benefits paid by your insurance. We accept many dental insurances, including some Blue Cross Blue Shield plans, Cigna, Delta, Dentemax, Humana, and United Concordia.
Once the carrier has paid the claim, any difference will be due upon receipt of our statement. We encourage you to contact us if your policy has not paid within 60 days.
