Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of service.

We offer various financial options to give the residents of Scottsdale, AZ the opportunity to have the smile you've always wanted.

You may choose from any of the following (including any combination) to pay your co-payment or full charge: Visa, MasterCard, American Express, debit card, cash, check or money order.

If you would like to extend your payments over a longer period of time, various financing plans can be arranged. Depending on the amount, these lines of credit are up to 6 months, interest- free.

We offer a 5% discount on charges that exceed $100 of the total amount due, if you pay in full at the first treatment visit.

Please ask a member of Dr. Clark’s staff, and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

Insurance

Our office understands the value of insurance benefits. We will gladly process and file your claim free of charge. Simply bring your estimated portion at the time of service. Please understand that this is only an estimate based on the information available to us. The final obligation for treatment is between you and our office. We will assist you in any way that we can; however, there are no guarantees and payment is due regardless of the benefits paid by your insurance. Once the carrier has paid the claim, any difference will be due upon receipt of our statement. We encourage you to contact us if your policy has not paid within 60 days.
